Like the fourth edition, this edition is thoroughly revised, with newsection editors and many new authors. Chapters have been rewritten andrestricted to reflect the new way diseases are diagnosed, categorized, andtreated. We have discarded out-of-date material and have added numerousnew items. Extra references and other material have been moved online tokeep the book itself as one volume
