The first part answers the question “What isHeart Failure?” The second and third parts address the clinical assessment of heartfailure and heart failure management including transitions of care and goals of care.The fourth part takes a closer look at heart failure and the comorbid conditions thatcan be challenging to manage in primary care. In each chapter, you will find at leastone case study as well as clinical pearls. An entire chapter is dedicated to addressingthe medications that should be avoided in the heart failure population. The fifth andfinal part explores challenging case studies. You will notice neither hypertension nordepression has specific chapters. The treatment of hypertension in heart failure ispatient-specific and usually addressed with the medications that are given for heartfailure. Depression is common for heart failure patients and should be treatedaccording to published guidelines for depression and practice site-specific protocols.
